Diabetes and your kidneys: the damage you cannot feel
Kidney function can fall a long way before anything hurts. The tests that catch it early are cheap, quick, and routinely not done.
Kidneys are patient organs. They lose a substantial amount of function before producing any symptom at all, and by the time something is noticeable the conversation has usually moved from prevention to management.
In diabetes this matters more than almost anywhere else, because raised blood sugar and raised blood pressure together are the most common route to kidney failure in this country.
The two tests worth asking for
A urine albumin-to-creatinine ratio detects small amounts of protein escaping into the urine. That leak begins years before creatinine rises and years before anything is felt — it is the earliest useful signal there is.
Alongside it, a serum creatinine with eGFR shows how well the filtering is being done. Between them they cost very little and take one visit, and in a great many people with diabetes neither has ever been done.
What protein in the urine means
It is not a sentence. It is an early warning, and it is the point at which tightening blood sugar and blood pressure control does the most good. Acted on, the trajectory often changes. Ignored, it tends not to.
Why self-medication is a particular risk here
Impaired kidneys clear substances more slowly, which changes how both medicines and herbal preparations behave in the body. Several preparations — including some sold freely for energy, weight or joint pain — are unsuitable in kidney impairment.

Established kidney disease belongs with a nephrologist. Work on blood sugar, blood pressure and diet sits alongside that care and never replaces it. Passing very little urine, breathlessness or sudden swelling need urgent assessment.
