What a metabolic disorder specialist actually looks at

Diabetes, fatty liver, high triglycerides, stubborn weight and high blood pressure usually arrive together. Here is why, and what an assessment covers.

Patients usually arrive naming one problem. The sugar. The weight. The fatty liver the ultrasound mentioned. In practice these are rarely separate illnesses that happened to arrive at the same address — they are one process showing itself in several places, which is why treating them one prescription at a time makes such slow progress.

The cluster

  • Raised blood sugar, or insulin resistance before the sugar rises
  • Weight carried around the middle rather than spread
  • Raised triglycerides and low HDL on a lipid profile

What an assessment covers

Measurements first, because without them nothing can be tracked: HbA1c, a lipid profile, liver and kidney function, blood pressure, and weight with the waist measured rather than guessed. A thyroid profile where the picture suggests it.

Then the part that takes longer — the routine that produced the numbers. What is eaten and, just as importantly, when. How the night goes. How much walking happens in an ordinary day, not a good one. What the work and the worry look like. These are not filler questions; they are where the cluster is built and the only place it can be unbuilt.

Why it is treated as one thing

Because a change that improves insulin resistance tends to move several of these at once. Weight off the middle shifts triglycerides, liver fat and blood pressure together. That is also why progress shows up across a set of numbers rather than in one — and why the review at four weeks looks at the set.

Can fatty liver be reversed?
